The registration form is below in 2 formats; the first is a Google Form and the 2nd is a fillable PDF. Please try to fill in the Google Form first and if that does not work, try the fillable PDF. If all else fails, come on into the school!

1) Make sure you have Adobe Acrobat Reader installed
2) Download the 2025-2026 Student Registration Form to your desktop.
3) Open the file on your computer using Adobe Acrobat Reader.
4) Fill in the form and then save it.
5) Email the completed form to: lkalinowski@srsd119.ca
